Established Patients:

If you have a concern in addition to the reason for your scheduled visitplease let our staff know when you arrive.

If you need to reschedule your appointment, please provide at least 24 hours’ notice, so we can afford the time to the other patients.

If there is a change in your insurance, let us know.

If you have any forms that need completion such as school physicals,handicapped placard forms, or employment forms please let us know when you schedule your visit and again when you check in.

If your health plan requires a co-payment, we will collect it during registration

Please note the last injection is given 30 minutes before closing time: all patients are required to wait for 20 minutes after the injection.
